Forms and policies to help students, staff and faculty navigate the guidelines of the Show Me Renewal plan:
- All students, faculty and staff are required to notify the university if they receive a positive COVID-19 diagnosis. Faculty, staff and student employees who test positive are expected to notify their supervisor along with their dean or division leader.
- Students must notify the MU Student Health Center within four hours of receiving their positive test results by submitting a secure online form, and they must notify their instructors they will not be attending class to make arrangements to continue their studies remotely, if they are able.
Students and faculty and staff should report behavior within the campus community that does not adhere to COVID-19 health protocols and expectations. Please fill out a form based on the role of the person exhibiting the behavior. Faculty and staff can also report an unsafe incident to their supervisor.
Supervisors should use the COVID-19 Tracking Form to report confirmed COVID-19 cases and potential COVID-19 exposure among faculty and staff. Supervisors, department chairs and division directors with staff and faculty working on-site are required to notify their university’s chief human resources officer (or designee) of an employee who has been diagnosed with COVID-19 or has had close contact with a person who has received a COVID-19 diagnosis.
COVID-19 requires all of campus to work together, so students, faculty and staff can be back on campus. Units are expected to pay for their own supplies such as masks, cleaning supplies, plexiglass, etc., however, the university is working to create cost efficiencies by placing bulk orders through The Mizzou Store. If a unit needs additional supplies not offered through bulk purchasing, a special request must be made and approved by the Incident Command Team.
Event organizers from campus organizations can request special approval for an event with more than 20 attendees by completing the COVID-19 Event Request Form. An exception will only be granted for events that support MU’s academic mission in which participants are required to adhere to strict social distancing, face covering requirements and specific protocols related to serving food and beverages.
As of Aug. 6, faculty or staff are no longer required to complete the Return to Work Authorization Form. However, undergraduate, graduate and professional students participating in research or creative activities — either as paid staff or in education roles — are required to submit a return to work authorization form to return to research or creative activities on campus.
