Forms to help students, staff and faculty navigate Show Me Renewal procedures:
Students, faculty and staff are strongly urged to get vaccinated and upload an image of their COVID-19 vaccination records or cards. Providing your vaccine information will help guide our policies to promote a safe working and learning environment. Vaccine information will be kept confidential.
- All students, faculty and staff are required to notify the university if they receive a positive COVID-19 diagnosis. Faculty, staff and student employees who test positive are expected to notify their supervisor along with their dean or division leader.
- Students must notify the MU Student Health Center within four hours of receiving their positive test results by submitting a secure online form, and they must notify their instructors they will not be attending class to make arrangements to continue their studies remotely, if they are able.
Supervisors should use the COVID-19 Tracking Form to report confirmed COVID-19 cases and potential COVID-19 exposure among faculty and staff. Supervisors, department chairs and division directors with staff and faculty working on-site are required to notify their university’s chief human resources officer (or designee) of an employee who has been diagnosed with COVID-19 or has had close contact with a person who has received a COVID-19 diagnosis.
Students, faculty and staff should report behavior within the campus community that does not adhere to COVID-19 requirements. Please fill out a form based on the role of the person exhibiting the behavior. Faculty and staff can also report an unsafe incident to their supervisor. Note that all are required to wear face coverings in classrooms or meeting spaces in which participants cannot socially distance. In other indoor spaces, face coverings are recommended for vaccinated and required for unvaccinated people. However, no one may inquire of individuals in those spaces about their vaccination status.
Checklists when notified of a COVID-positive employee or student.
