Forms to help students, staff and faculty navigate Show Me Renewal procedures:
- All students, faculty and staff are required to notify the university if they receive a positive COVID-19 diagnosis. Faculty, staff and student employees who test positive are expected to notify their supervisor along with their dean or division leader.
- Students must notify the MU Student Health Center within four hours of receiving their positive test results by submitting a secure online form, and they must notify their instructors they will not be attending class to make arrangements to continue their studies remotely, if they are able.
Supervisors should use the COVID-19 Tracking Form to report confirmed COVID-19 cases and potential COVID-19 exposure among faculty and staff. Supervisors, department chairs and division directors with staff and faculty working on-site are required to notify their university’s chief human resources officer (or designee) of an employee who has been diagnosed with COVID-19 or has had close contact with a person who has received a COVID-19 diagnosis.
