Forms to help students, staff and faculty navigate Show Me Renewal procedures:

Student Self-Report Form

  • All students, faculty and staff are required to notify the university if they receive a positive COVID-19 diagnosis. Faculty, staff and student employees who test positive are expected to notify their supervisor along with their dean or division leader.
  • Students must notify the MU Student Health Center within four hours of receiving their positive test results by submitting a secure online form, and they must notify their instructors they will not be attending class to make arrangements to continue their studies remotely, if they are able.

COVID-19 Tracking Form

Supervisors should use the COVID-19 Tracking Form to report confirmed COVID-19 cases and potential COVID-19 exposure among faculty and staff. Supervisors, department chairs and division directors with staff and faculty working on-site are required to notify their university’s chief human resources officer (or designee) of an employee who has been diagnosed with COVID-19 or has had close contact with a person who has received a COVID-19 diagnosis.
